Russian inmate Boyka, now severely hobbled by the knee injury suffered at the end of Undisputed 2. No longer the feared prison fighter he was, he has declined so far that he is now good only for cleaning toilets. At an exclusive country club, an ambitious young caddy, Danny Noonan, eagerly pursues a caddy scholarship in hopes of attending college and, in turn, avoiding a job at the lumber yard. In order to succeed, he must first win the favour of the elitist Judge Smails, and then the caddy golf tournament which Smails sponsors.

The vicious, power-mad Kano is determined to take over Earthrealm, one soul at a time. Assisted by a trio of cold Black Dragon mercenaries, he embarks on a brutal assault from town to defenseless town. The choice is simple: kneel or be annihilated. Kenshi, a rebel warrior, calls on Kuai Liang, the only one powerful enough to challenge the malevolent Kano. After deciding he needs to do something meaningful with his life, high school wrestler Louden Swain sets out on a mission to drop weight and challenge the area's undefeated champion, which creates problems with his teammates and health. Matters are complicated further when Louden's father takes in an attractive female drifter who's on her way to San Francisco.

Raised as a slave, Danny is used to fighting for his survival. In fact, his "master," Bart, thinks of him as a pet and goes as far as leashing him with a collar so they can make money in fight clubs, where Danny is the main contender. When Bart's crew is in a car accident, Danny escapes and meets a blind, kindhearted piano tuner who takes him in and uses music to free the fighter's long-buried heart.
